Why the date alone is not enough
The date gives the Sun's sign and roughly the Moon's. The time and place give the lagna and the houses, and the houses are what the text reads for each part of life. Two people born on the same day in the same city, a few hours apart, have different lagnas and so different readings. That is why the form asks for all three.

Is this the same as a sun-sign horoscope?
No. Sun-sign horoscopes use only the Sun's sign. A Vedic reading uses the whole chart: the lagna, all nine planets, their houses, signs, strengths and periods.
How accurate is it?
The planet positions come from NASA JPL's DE440 tables, which observatories use. The readings are exactly what the texts say for your chart, with the verse shown; whether the texts are right about people is for you to judge. Astrology is not scientifically proven.
